Applicants are asked to complete the following form. Pressing the 'Submit Registration Info' button at the bottom will cause the entered information to be sent to our treasurer, Thomas Keller (thomas.keller@sympatico.ca), for processing. Applicants still need to send a 'regular mail' cheque to Thomas to complete the registration process. Please ensure your name is clearly shown on the cheque, along with 'OFA Membership 2007' on the cheque's 'memo' line if possible. Cheques are to be mailed to:
